Keep the survey. Lose the form.

Send the survey you already have. Simple Survey turns the same questions into a shorter text or voice conversation and returns the fields you asked for with the source context attached.

Why we built it

A form makes the respondent do too much of the research team’s work.

Before someone can share an experience, a conventional form asks them to open a link, understand the interface, decode every question, fit their answer into predetermined boxes, and often finish in one sitting.

We built Simple Survey to keep the organization’s questions and structured output while giving the person a more natural way to answer.

The survey is still rigorous. The burden moves away from the person answering it.

What it does

One answer can cover several questions. The next question changes with what is missing.

The organization supplies the survey goal, required answer fields, approved context, consented audience, language and reminder rules, and the places where a person must review the result.

Simple Survey maps each response to the questions it already answers. It asks a follow-up only when required information is still missing, then returns the structured fields with the original conversation available for traceability.

From questions to usable answers The same survey takes a shorter route through the person’s experience.
  1. Survey as-isQuestions, fields, consent, and approved context
  2. Open conversationA natural reply by text or voice
  3. What is missing?Map what is answered and ask one useful follow-up
  4. Reviewable outputStructured fields and source context stay together

Who it helps

Less work for the person. Better evidence for the team asking.

For the person answering

Respond in your own words and your own time.

A person can answer naturally, pause, continue later, and respond through the channels agreed for the survey.

  • Fewer repeated questions
  • Text or voice in the agreed language
  • No new survey portal to learn

For the organization

Keep the fields. Keep the source story too.

The team receives the structured answers it planned for and can inspect the source conversation when nuance or judgment matters.

  • Existing survey questions and fields
  • Structured output for the agreed destination
  • Source messages, transcripts, and review context

Why it is different

The conversation adapts. The research contract stays visible.

The product does not begin by asking the organization to invent a new survey system. It begins with the questions and output already required.

Same input

Bring the survey you already have.

The product starts from the existing questions, required fields, audience, and decision—not a new form builder.

Adaptive work

Ask what this person still needs to answer.

A rich response can fill several fields, so the next question can focus on the remaining gap.

Evidence preserved

The structured row is not the only record.

The source conversation stays available so important interpretation can be checked rather than treated as invisible automation.

Why Powered by Polyform

A simple conversation depends on maintained meaning and visible evidence.

Simple Survey is one example of how Polyform turns a focused problem into a complete product path. The surface is a conversation. Underneath it are the organization’s questions, consented context, changing evidence, structured output, review, and a human decision boundary.

Powered by Polyform means the product was conceived, designed, and built as part of the Polyform product family. It is not a claim that every response runs inside the Polyform application.
